NHMHCA 2016 Spring Workshop Online Registration

Registrations should be received by 4/1/2016. Seating is limited and registration will be accepted on a first-come first-served basis. Please give us notice of any ADA requirements prior to 4/1/2016. CANCELLATIONS received in writing prior to 4/1/2016 will receive a full refund. 

If you want to get the group discount you must print a printable form (below) for each person in your group and mail it in with your check(s) to the address below.

Online Registration has two steps. Please remember to complete both steps:

1) First, go to the online registration form here. Fill out the form and press submit.

2) Come back to this page and pay using the PayPal button below. You do not have to have a PayPal account to pay through PayPal, nor do you have to create one.
